On May 6th, a Virgin Atlantic flight bound for New York from London was 40 minutes into their flight before having to turn back due to a “rostering issue” with the flight’s first officer. Many public comments chastise both the first officer and their lines for this egregious oversight, but was the “oops” a lack of awareness or was it the result of making ends meet on an already burnt-out system? 

What do you think, should the first officer have taken extra steps before boarding the flight? What steps should be implemented to prevent this from happening again? Would extra steps making other operations more difficult?
