The Ministry of Education admits it should have done more to test the air at a Lower Hutt high school where toxic mould has hugely disrupted teaching.
But the admission in a new review does not reveal why officials did not move faster when alerted in 2019.
Building checks have found the roof of an entire block of 17 classrooms at Hutt Valley High is a "fungal reservoir" releasing unhealthy spores at 27 times acceptable levels.
