Several crash investigators have left the Transport Accident Investigation Commission (TAIC), with some blaming a toxic culture and questioning whether the agency is up to the job.
Over the past five years, there's been an almost complete overhaul of the investigations team and staff say they fear the loss of so many experienced people will have public safety repercussions.
Minister Michael Wood has asked the Ministry of Transport to look into what's happening.
TAIC chief executive Lois Hutchinson spoke to Susie Ferguson.
