Since its launch in 2019, Ottawa's new LRT system has encountered more and more difficulty - from malfunction to derailment, to investigations into what went wrong. This week, Canada's Transportation Safety Board concluded that "inconsistent and incomplete maintenance" was to blame for the O-Train derailment in September - an incident that had shut down the LRT system for almost two months. It's back on track now, but the Ontario government has announced a public inquiry into what's gone wrong with the $2.1 billion project.
